Mission Statement
To empower minority single and pregnant mothers through accessible, culturally relevant prenatal care, mental health services, and childcare support, helping them achieve healthier outcomes for themselves and their children.

Program Goal

Improve the health and well-being of minority single and pregnant mothers by increasing access to prenatal care, mental health services, and resources for childcare by the end of the program period.

SMART Goals

 Process Objectives

• Organize four community health workshops on prenatal care, mental health, and childcare resources, with participation from OB/GYNs, social workers, and public health nurses, by the end of the first six months.

• Distribute at least 300 surveys across clinics, community centers, and advocacy groups to assess prenatal care needs and childcare challenges within the first 90 days of program launch.

• Develop partnerships with two minority health advocacy groups and three local healthcare providers to ensure culturally appropriate services are incorporated by the third month.

Outcome Objectives

• Short-term Objectives

                               o By the fourth month, at least 50% of the program participants will report increased knowledge about the importance of early prenatal care based on survey responses collected at the end of each health workshop.

                               o Increase enrollment in prenatal care services by 10% within the first three months of the program through outreach efforts and referral partnerships with OB/GYN clinics

• Intermediate Objectives

                               o Within six months, 60% of program participants will have attended at least one mental health counseling session offered through the program's partnership with local providers.

                               o Increase access to affordable childcare services by facilitating connections to local childcare programs for 40% of program participants within the first eight months.

• Long-term Objective

                               o Within two years, reduce the rate of low birth weight among infants born to participating mothers by 15% compared to baseline data collected at program enrollment
